Dance Permission Forms
All students who wish to attend a dance will need to fill out a Dance Permission form showing parent approval. Only one form is needed for the school year. Access form below.
AGREEMENT FOR ACTIVITY PARTICIPATION
All sections of this Agreement must be completed, with the signed original delivered to the School Office, before a
Student will be allowed to participate in any manner in the Activity defined below such as particiaption on inflatables.
